Parent or guardian consent

Give permission for Hand of Emmanuel to consider mentorship for your child and contact you. For ages 13–17, this is initial permission to discuss and assess the opportunity, not final consent to participate.

Sessions are planned online; in-person meetings may be arranged based on mentor and mentee availability. We will explain the arrangements and safeguards before seeking final consent.

Before any match, we will explain the activities, supervision, and safeguards, verify your identity, and ask for final consent. No mentor will be introduced to your child on the basis of this form alone.
