For ages 13–17, a parent or legal guardian must complete this form. Young adults aged 18 and older may apply for themselves. We will contact you when mentoring arrangements are established.

Session format

Mentorship sessions will take place online. In-person sessions may be arranged based on mentor and mentee availability, with parent or guardian agreement and the program’s safeguarding measures.
